Job Title: Support Worker
Location: Birmingham
Reports to: Team Leader / Service Manager
Hours: Full-time / Part-time / Shifts (including evenings, weekends, and sleep-ins as required)
Job Purpose:
To provide person-centred care and support to individuals living in supported living accommodation, empowering them to live as independently as possible, while ensuring their health, safety, and emotional well-being. You will support service users in all aspects of daily living, helping them to make informed choices and participate fully in their community.
Key Responsibilities:
1. Person-Centred Support:
- Support individuals with daily tasks such as cooking, cleaning, shopping, personal care, and medication.
- Promote independence, dignity, and choice in all aspects of life.
- Help service users to set and achieve personal goals, including education, employment, and social activities.
2. Health, Safety, and Risk Management:
- Ensure a safe and clean living environment.
- Follow care plans and risk assessments tailored to each individual.
- Monitor and report changes in physical or mental health to senior staff or external professionals.
3. Communication and Record Keeping:
- Maintain accurate records of support provided, incidents, and significant events.
- Communicate effectively with colleagues, families, and health/social care professionals.
- Respect confidentiality and follow data protection protocols.
4. Safeguarding and Compliance:
- Uphold safeguarding principles and report concerns in line with policy and procedure.
- Adhere to all organisational policies and procedures, including health and safety, equality and diversity, and professional boundaries.
5. Teamwork and Professional Development:
- Work collaboratively with other support workers, managers, and external agencies.
- Attend supervision, training sessions, and team meetings.
- Contribute to service improvement by sharing ideas and feedback.
Essential Requirements:
- Compassionate, patient, and committed to supporting vulnerable individuals.
- Ability to work independently and as part of a team.
- Good communication and organisational skills.
- Basic literacy and numeracy.
Desirable Qualifications and Experience:
- Previous experience in a care or support setting.
- NVQ Level 2 or 3 in Health and Social Care (or willingness to work towards it).
- Knowledge of safeguarding, mental health, learning disabilities, or autism.
- Full UK driving licence (if required by the service).
